The Neuro Therapy Centre (NTC) is a specialised provider of support for people with neurological conditions in the voluntary sector. As part of our lottery funded project to improve access to exercise and wellbeing services for people with neurological conditions we have the opportunity to introduce this role to encourage referrals and promote opportunities for access.
Working under the supervision of a Neuro Therapy Centre Lead the role will provide social prescribing that empowers people to take control of their health and wellbeing and shift the focus back to what matters to them. Taking a holistic approach to the health and wellbeing of an individual living with a neurological condition, connecting people to community groups and statutory services for practical and emotional support.
You will work as a key part of the primary care network (PCN) multi-disciplinary team, helping PCNs to strengthen community and personal resilience and reducing health and wellbeing inequalities by addressing the wider determinants of health, such as physical inactivity, mental & emotional wellbeing, and signposting to other agencies where appropriate to increase people’s active involvement with their local communities.
