We are a member-led charity which puts its members first. By joining you will become part of our supportive, friendly community, whether you are someone with a neurological condition who's looking for advice, a listening ear or help to manage your condition; a Carer of someone with a neurological condition; or you want to become a Friend of the Centre to support our work. Membership runs annually on a rolling basis from your join date. People with Neurological Conditions and Carer Members Our self-care, listening and peer-support services are available to our members on joining. If you are interested in accessing our exercise and physiotherapy services, you will be invited by our physiotherapy team to have a medical assessment. This is so we can tailor our advice and support to your individual needs. You will find a section in our membership form relating to the services you're interested in accessing. Our services and support are for people with neurological conditions, please check the list of conditions we support. If you think our support may be relevant to you, but your condition isn't listed, please get in touch by calling 01244 678619, or contacting us here. We understand that caring for someone with a neurological condition can be difficult therefore the Centre provides specific help for those caring for people with neurological conditions. How do I join? Please complete the relevant online Membership Form(s) below: Person with a Neurological Condition Membership Form Carer Membership Form Friend Membership Form We encourage people to join and pay for their membership online where possible. When you submit your online form you will be directed to the online payment page. There is an option for person with a neurological condition membership (fee £26.40), Carer membership (fee £13.20), and the ability to pay these together (fee £39.60), as well as the option for Friend membership (fee £11) to support the Centre. We encourage people to join online where possible. if it is not possible for you to pay online please contact us to discuss joining through the Centre on 01244 678619 or at [email protected] Paying Annually by Direct Debit If you would like your membership payment to be created as a direct debit which will be taken annually on the anniversary of your chosen payment date, then please click the Regular button instead of the One-Off option at the top on the right side. Your payments are protected by the Direct Debit Guarantee. The Guarantee is offered by all banks and building societies that accept instructions to pay Direct Debits. If there are any changes to the amount, date or frequency of your Direct Debit you will be notified in advance of your account being debited or as otherwise agreed. Confirmation of the amount and date will be given to you at the time of the request. If an error is made in the payment of your Direct Debit, by us or your bank or building society, you are entitled to a full and immediate refund of the amount paid from your bank or building society. If you receive a refund you are not entitled to, you must pay it back when we ask you to. You can cancel a Direct Debit at any time by simply contacting your bank or building society. Written confirmation may be required. This will mean that your membership of the Neuro Therapy Centre will lapse and you will no longer be able to access our services once your membership year has expired, but you are of course welcome to re-join at any point. Can I make a donation when I join? Donations can also be added when you pay your membership and these are very much appreciated. Please select 'For additional donation' as the amount and add your donation to the membership fee you owe. As a charity donations contribute significantly to the running of the Centre and allow us to continue to support all those who attend. As both membership and donations can be gift-aided, please tick the gift-aid box if you pay online and are eligible. This really makes a difference as it means that we can claim an extra 25% on your payment for free an at no additional cost to you. To be eligible you must be a tax payer and pay tax over your donation amount. Your assessment for physiotherapy and exercise To access our physiotherapy and fitness sessions, you will be given an opportunity to get to know the Centre and have an assessment so that the therapy sessions that are most suitable to you are recommended. You will then be asked to pay your membership fee and join either online or at the Centre, so that you are able to book on these therapy sessions and you will become a member. We undertake face-to-face assessments at the Centre in Saltney and can also offer a virtual assessment if that is a better option for you. Your medical records We will ask you to give your consent for us to get in touch with your GP to request your relevant medical records ahead of our individual assessment with our physiotherapy team if you would like to access our physiotherapy/exercise services, so that you can start exercising and receiving therapies from us. If your GP does not have our referral form to hand, one can be downloaded here. Our physiotherapists use this information to help us recommend which of our services will be best for you.
